Dr. Samuel Lee is a native of Fort Worth, attending Eastern Hills High School and then Texas Wesleyan University. He graduated from the University of Osteopathic Medicine and Health Sciences in 1994 and completed his residency in Family Practice here in Fort Worth in 1997.
He has been in private practice since 2000 and enjoys encouraging his patients to take control of their health and educations them on how to improve their quality of life.
Dr. Lee and his staff have been involved in the Hispanic community for over 20 years including mission trips to Mexico and Central America.
When not in the office he enjoys spending time with his family, traveling, reading, and collecting political campaign buttons.
Candice Moore FNP-C, is a Family Nurse Practitioner certified by the American Academy of Nurse Practitioners. Candice grew up in Northwest Arkansas and spent the latter part of her childhood in Austin, Texas. She earned her Bachelors of Science degree in Nursing at the University of Mary Hardin-Baylor in 2008. She served as a bedside nurse, striving to usher her patients back to health in the medical intensive care unit. She later relocated to Dallas where the cardiovascular intensive care unit became her home. After 8 years of aiding in the recovery of open heart surgical patients, she obtained her Masters of Science in Nursing as a Family Nurse Practitioner.
Candice provides care for patients of all ages. She enjoys bonding with and treating patients with chronic and acute diseases. In her free time she enjoys spending time with her husband and dog, international travel, exercising and watching movies.
Melissa Encinas, PA-C was born and raised in and around Fort Worth, Texas. She graduated with a Master's in Physician Assistant Studies from the University of North Texas Health Science Center in 2007. She is certified as a physician assistant by the National Commission on Certification of Physician Assistants and licensed by the Texas Medical Board. She has been practicing family medicine at the Diamond Hill Medical Center since 2007.
Melissa is fluent in English and Spanish. Her interests include nutrition, diabetes management, and hormone optimization. In her spare time, she enjoys spending time with family, reading, listening to music and podcasts, exercise, and meditation.
Joel Munoz PA-C grew up in the small town of Ferris, Texas. He attended the University of Mary Hardin-Baylor and graduated in 2008 with a Bachelor of Science in Biology. While at UMHB, he was the kicker for an astounding football team and earned All-American Honors.
After undergrad, Joel worked in the Pathology lab at Scott & White Hospital in Temple, Texas. He then attended UNT Health Science Center and earned his Master of Physician Assistant Studies in 2013. Since that time he has worked as a PA at Diamond Hill Medical Center and enjoys the privilege of being able to serve the Hispanic community and motivate patients to improve their health holistically.
In his spare time, Joel enjoys spending time with family and friends, working out, playing soccer, and attending music festivals.
